The Site is not medical advice

Content on this Site is provided for general informational purposes only and is not medical advice, diagnosis, or treatment. Using the Site, booking a consultation, or emailing us does not create a provider-patient relationship. A provider-patient relationship is established only after you complete intake and are accepted as a patient following an individual evaluation.

Do not use this Site for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.

Services and eligibility

LIFE NP provides telehealth services only to adults (18+) physically located in states where our providers are licensed at the time of service (currently Arizona and North Dakota). Membership fees cover clinical services only. Any prescription is issued solely at the discretion of the treating provider; payment of a fee does not guarantee that any medication will be prescribed. Medication, laboratory, and pharmacy costs are separate unless otherwise stated.

Memberships and cancellation

Memberships are billed monthly and may be canceled at any time, effective at the end of the current billing period. We reserve the right to modify pricing with advance notice.

Acceptable use

You agree not to misuse the Site, attempt to gain unauthorized access to any systems, or use Site content to impersonate LIFE NP.

Intellectual property

The Site and its content, including the LIFE NP name, logo, and text, are owned by LIFE NP and protected by applicable law. You may not reproduce them without our written permission.

Disclaimers and limitation of liability

The Site is provided "as is" without warranties of any kind. To the fullest extent permitted by law, LIFE NP is not liable for indirect, incidental, or consequential damages arising from use of the Site. Nothing in these terms limits liability that cannot be limited under applicable law, or affects the standard of care owed to established patients.

Governing law

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles.
